A Social Prescriber can support you with practical, social and emotional issues that may be affecting your wellbeing. They take the time to listen, understand what matters to you, and help connect you with local services, groups and support available in the community.

How a Social Prescriber Could Help

A Social Prescriber can help connect you to local services, groups and support for:

·         Loneliness and social isolation

·         Emotional wellbeing

·         Caring responsibilities and support for carers

·         Living well with long-term health conditions

·         Housing concerns

·         Financial worries

·         Accessing local groups, activities and community support

·         Bereavement and life-changing events

·         Employment, volunteering and education opportunities

·         Building confidence, independence and social connections

·         Healthy lifestyle goals and wellbeing support

Your Social Prescriber will work with you to understand your needs and goals and help you access the support that is right for you.

How to Access the Service

You can contact the Leatherhead Primary Care Network Social Prescribing Team on 01306 771125 (option 2) or make a self-referral online at services.thejoyapp.com/en/listings/12725

Alternatively, speak to your GP, nurse, or another healthcare professional, who can refer you to the service.
